Consultations are $100. If you are considering a procedure that may be partially covered by insurance, you will be responsible for any required co-pays or co-insurance.
Insurance
At Holzapfel & Lied Plastic Surgery, we understand that having plastic surgery can be a major financial commitment. We are proud to participate with most major insurance companies for qualifying procedures. These include but are not limited to reconstructive surgery; deviated septum repair; and the removal of cysts, moles, and lesions. If you have questions about these procedures, or about insurance benefits that your coverage offers, please contact our office.
To reduce your out-of-pocket payment, please verify that our physicians are in your insurance network.
Surgery Policy
The fees that are indicated are for the procedures listed below. These fees will be honored for one year from today's date. All fees are payable three weeks prior to the scheduled date of the procedure. Fees may be paid by cash, check, Visa, MasterCard, American Express or Discover.
A 10% deposit is required to hold any date for surgery and the deposit is deducted from the surgeon's total fee. The balance is due three weeks in advance of the scheduled surgery date.
Once you have been scheduled for surgery, we request that you make every effort to avoid canceling or re-scheduling. If you cancel within 3 weeks of your surgery, the 10% deposit will not be refunded. If you cancel within 2-12 days of surgery, 25% will not be refunded. Cancellation of surgery within 48 hours of surgery, 50% is non-refundable. This policy has been adopted to ensure the best patient care and to protect your and other patients' reservations on our surgery schedule.
On occasion, some patients require revision surgery. If the revision is scheduled during the first year following surgery, the surgeon may waive his fees. However, a fee for facility and anesthesia must be paid 3 weeks prior to surgery.
If deposit to hold a date is made and a change is made, there will be a $500.00 change fee.
If there is a cancellation before 3 weeks, there is a $500.00 NON-REFUNDABLE fee.
NO refund of the deposit if any changes or cancellation within the 3 weeks.
This quote is being created to affirm that the Doctor is performing the procedure for cosmetic reasons, and that this procedure is NOT medically necessary.
